RESUMO

There are various pathologies that involve the hallux, among which hallux valgus is the most common. When conservative treatment fails, it is necessary to resort to a surgical approach. The fixation elements for osteotomies in the hallux are usually composed of metallic materials; however, today, there are numerous resorbable materials that offer numerous advantages over conventional materials. In this article, the objective is to analyze the scientific evidence through the systematic analysis of the existing literature in relation to the effectiveness of resorbable versus non-resorbable osteosynthesis material in the surgical correction of hallux deformities and compare the complications as well as the patient satisfaction and quality of life between both fixation methods. A systematic review of the literature available in the PubMed, EMBASE, Web of Science and Scopus databases and 10 studies were included. The documents were chosen following the eligibility and exclusion criteria, including experimental and observational studies evaluated with the Jadad and Newcastle-Ottawa methodological quality scale, respectively. Data were extracted from valid studies for the review, and the variables functionality, pain, angular corrections, complications, satisfaction and quality of life were observed. In conclusion, there is limited scientific evidence regarding the effectiveness of resorbable versus non-resorbable osteosynthesis material in the surgical correction of hallux deformities. All observed variables are similar regardless of the surgical technique and osteosynthesis material used.

The aim of the present study was to identify the risk factors for removal of osteosynthesis material after multi-piece Le Fort I osteotomy compared to standard one-piece Le Fort I osteotomy (LF1). Medical files of patients treated with multi-piece or one-piece LF1 were retrospectively reviewed, including the indication for removal and time between insertion and removal. A total of 339 patients were included: 290 patients with LF1 and 49 patients with multi-piece LF1. Patients undergoing multi-piece LF1 had 2.7-times significantly higher (p < 0.001) relative risk of osteosynthesis removal in the upper jaw (42.9%) than patients undergoing LF1 (15.9%). Significant independent predictors of removal of osteosynthesis material after multi-piece LF1 were older age (odds ratio [OR] 1.1, 95% confidence interval [CI] 1.0-1.2; p = 0.028), simultaneous bilateral sagittal split osteotomy (OR 7.8, 95% CI 1.2-50.3; p = 0.031), and no previous surgically assisted rapid palatal expansion (OR 0.14, 95% CI 0.03-0.69; p = 0.15). Significantly higher removal rates of osteosynthesis material were found after multi-piece LF1. Therefore, all patients must be informed of the higher risk for removal of osteosynthesis material when undergoing a multi-piece LF1.

RESUMEN Introducción: la fractura de cadera es una de las causas más frecuentes de cirugía traumatológica. La selección del material de osteosíntesis constituye el objetivo fundamental en el tratamiento quirúrgico, lo que puede influir en el nivel de validismo alcanzado. Objetivo: evaluar el nivel de validismo en los pacientes intervenidos quirúrgicamente, con diferentes implantes según los diferentes tipos de fracturas del extremo proximal de fémur. Atendidos en el Hospital Provincial Clínico Quirúrgico "Comandante Faustino Pérez Hernández" de la provincia de Matanzas. Materiales y métodos: se realizó un estudio prospectivo, longitudinal donde se estudiaron todos aquellos pacientes con fractura de cadera. En el periodo comprendido desde el 1er de enero del 2016 al 31 de diciembre del 2017. Resultados: predominó el sexo femenino con un 60 % y el grupo de edades de 70 a 79 años para ambos sexos. Prevaleció la fractura extracapsular en un 74 %. El material de osteosíntesis más representado fue el clavo intramedular Gt y en las intracapsulares la prótesis simple. El tiempo quirúrgico se comportó con un tiempo menor a 2 h y el egreso antes de los 5 días. El 48,2 % de los pacientes fueron evaluados de bueno según nivel de validismo alcanzado posterior al tratamiento aplicado. Conclusiones: la fractura de cadera es propia de pacientes adultos mayores que sobrepasan los 65 años. La intervención quirúrgica de urgencia con una adecuada selección del método de osteosíntesis, disminuye un serio problema de salud en el contexto médico, social y epidemiológico cubano (AU).


ABSTRACT Introduction: hip fracture is one of the most frequent indications of trauma surgery. The choice of the osteosynthesis material is the main objective in the surgical treatment, which can influence the level of validity achieved. Objective: to assess the level of validism in patients who underwent surgery with different implants according to the different types of fracture of the proximal end of the femur in the Provincial Surgical Clinical Hospital Comandante Faustino Pérez Hernández of the province of Matanzas. Materials and methods: A prospective, longitudinal study was carried out in all patients with hip fracture in the period from January 1st 2016 to December 31st 2017. Results: female sex (60%) and the 70-79 age group for both sexes (36%) predominated. The extracapsular fracture prevailed with 74%. The most represented osteosynthesis material was the Gt intramedullary nail, and the simple prosthesis in the intracapsular ones. Surgical time was less than 2 hours and the patients were discharged before the 5th day. 48.2% of patients were evaluated as good according to the level of validism achieved after the applied treatment. Conclusions: hip fracture is proper of elder adult people aged more than 65 years. The emergency surgical intervention with an adequate choice of the osteosynthesis method, reduces a serious health problem in the Cuban epidemiological, social and medical context (AU).

OBJECTIVES: To compare the results of secondary alveoloplasty performed in one Hospital when osteosynthesis material was used and when the bone graft does not require this material, and relating them to factors such as gender and age. MATERIAL AND METHODS: A retrospective study was conducted from the years 2014 to 2019 in this Hospital on the selected patients who met the inclusion criteria. Two periods of ages, period A: ages between 5-12 years (mixed secondary alveoloplasty) and period B: greater than 12 years (late secondary alveoloplasty). Autologous bone from the iliac crest or parietal calotte was used for the bone graft. The patients were divided into 2 groups: group I: patients with alveoloplasties that required osteosynthesis material. Group II: patients who did not require osteosynthesis material. Parameters evaluated: the success criteria for alveoloplasty were assessed according to the clinical parameters described by Precious. Alveoloplasty was successful if they met all the criteria of Precious in the year of intervention. Postoperative complications in both groups were evaluated. The statistical analysis was performed using the exact Fisher test for qualitative variables. RESULTS: Alveoloplasty was successful in 89.4% of patients in group I, while it was 90.3% in group II. Alveoloplasty was successful in 87.5% of females compared to 91.17% of males. The intervention was a success in 91.48% of patients in group A, compared to 66.6% in group B. The osteosynthesis material in two patients of group I was not degraded in the annual assessment. There were no significant differences in any of the comparisons. CONCLUSIONS: The use of osteosynthesis material does not alter the integration of the bone graft in patients that undergo alveoloplasty. Factors such as gender or age do not influence the results of the interventions.

Aim To calculate stress and deformation under the force of pressure and bending in the dynamic compression plate (DCP), locking compression plate (LCP), selfdynamisable internal fixator (SIF) and locked intramedullary nail (LIN) in the models of juvidur, beef tibia bone (cadaver) and software of bone model simulator. Methods Juvidur and bone models were used for the experimental study, static tests were performed with SHIMADZU AGS-X tester. CATIA software was used to create a 3D model for the SCA simulator, while software ANSYS to calculate the tension and deformation for compressive and bending forces. Stress and deformation analysis was performed with the use of Finite Element Analysis (FEA). Results Weight coefficients of research methods were different (juvidur=0.3; cadaver=0.5; SCA Simuator=0.2), and weight coefficients of the force of pressure Kp =0.5 and bending forces in one plane K1 =0.25 and K2 =0.25 in another plane, the overall result on the dilatation of DCP, LCP, LIN and SIF on juvidur and veal cadaver models showed that the first ranking was the LIN with a rank coefficient KU-LIN = 0.0603, followed by the IFM with KU-IFM = 0.0621, DCP with KU-DCP = 0.0826 and LCP with KU-LCP = 0.2264. Conclusion Dilatation size did not exceed 0.2264 mm, hence the implants fulfilled biomechanical conditions for the internal stabilization of bone fractures. Prevalence goes to the locked intramedullar nailing and Mitkovic internal fixator in the treatment of diaphyseal, transversal, comminuted fractures in relation to DCP and LCP.

RESUMO

Post-operative spondylodiscitis due to Propionibacterium acnes is rare, with only a few cases reported in the literature. In this article, we report the case of a 76-year-old patient who suffered from spondylodiscitis T12-L1 discovered after 1 month of pain and fever. Antibiotic treatment was administered for 6 weeks without revision surgery. Diagnosis remains incomplete, and preventive therapy against this infection is not well known. There is no link between immunosuppression and this infection. A cure has been reported in 98% of cases, but further investigation is still required for the establishment of treatment guidelines.

BACKGROUND: An implant used for stabilizing a fracture creates a mechanical construct, which directly determines the biology of bone healing. The stabilization of fractures places high mechanical demands on implants and therefore steel and titanium are currently almost exclusively used as the materials of choice. OBJECTIVES: The possible range of attainable mechanobiological stimulation for mechanotherapy as a function of plate stiffness depending on the selection of the plate material and the physical and mechanical properties of the material options are discussed. MATERIAL AND METHODS: An overview of the material properties of steel and titanium is given. For dynamically fixed long bone fractures as examples, various finite element models of plate osteosynthesis (steel/titanium) are created and the plate working length (PWL, screw configuration close to fracture) is varied. The interfragmentary movement (IFM) as a measure of mechanobiological stimulation is evaluated. RESULTS: Stimulation in the form of IFM varies across the fracture and also as a function of the osteosynthesis material and the configuration. The influence of the material appears to be notably smaller than the influence of PWL but both lose their influence largely over a bridged fracture situation (contact). With a flexible titanium plate and large PSS, a greater mechanobiological stimulation is produced. CONCLUSION: An essential prerequisite for the secondary fracture healing is an appropriate mechanobiological environment, which can be controlled by the osteosynthesis material and the configuration and is also affected by the type of fracture and load.

INTRODUCTION: Artificial models can be useful at approximate and qualitative research, which should give the preliminary results. Artificial models are usually made of photo-elastic plastic e.g.. juvidur, araldite in the three-dimensional contour shape of the bone. Anatomical preparations consist of the same heterogeneous, structural materials with extremely anisotropic and unequal highly elastic characteristics, which are embedded in a complex organic structure. THE AIM OF THE STUDY: Examine the budget voltage and deformation of: dynamic compression plate (DCP), locking compression plate (LCP), Mitkovic internal fixator (MIF), Locked intramedullary nailing (LIN) on the compressive and bending forces on juvidur and veal bone models and compared the results of these two methods (juvidur, veal bone). MATERIAL AND METHODS: For the experimental study were used geometrically identical, anatomically shaped models of Juvidur and veal bones diameter of 30 mm and a length of 100 mm. Static tests were performed with SHIMADZU AGS-X testing machine, where the force of pressure (compression) increased from 0 N to 500 N, and then conducted relief. Bending forces grew from 0 N to 250 N, after which came into sharp relief. RESULTS: On models of juvidur and veal bones studies have confirmed that uniform stability at the site of the fracture MIF with a coefficient ranking KMIF=0,1971, KLIN=0,2704, KDCP=0,2727 i KLCP=0,5821. CONCLUSION: On models of juvidur and veal bones working with Shimadzu AGS-X testing machine is best demonstrated MIF with a coefficient of 0.1971.

The most common complications that are associated with bilateral sagittal split osteotomy are: bad splits, postoperative infection, removal of osteosynthesis material, and neurosensory disturbances of the lower lip. Particularly in elective orthognathic surgery, it is important that surgeons inform their patients about the risk of these complications and attempt to minimize these risks. The purpose of this literature review and meta-analysis is to provide an overview of these common complications and their risk factors. After a systematic electronic database search, 59 studies were identified and included in this review. For each complication, a pooled mean incidence was computed. Both the pooled study group and the pooled 'complication group' were analysed. The mean incidences for bad split (2.3% per SSO), postoperative infection (9.6% per patient), removal of the osteosynthesis material (11.2% per patient), and neurosensory disturbances of the lower lip (33.9% per patient) are reported. Regularly reported risk factors for complications were the patient's age, smoking habits, presence of third molars, the surgical technique and type of osteosynthesis material. This information may help the surgeon to minimize the risk of these complications and inform the patient about the risks of complications associated with bilateral sagittal split osteotomy.

INTRODUCCIÓN: Las infecciones osteoarticulares son un desafío frecuente en la práctica traumatológica diaria. En Chile y en los distintos hospitales de la región de Valparaíso son escasos los trabajos con relación al tema, lo que es fundamental para optimizar el tratamiento, tanto quirúrgico como médico, de estas infecciones. OBJETIVOS: Describir las infecciones osteoarticulares del Hospital del IST de Viña del Mar durante el periodo 2012-2013. MATERIALES Y MÉTODO: Trabajo descriptivo retrospectivo de una serie de casos. Se revisaron las fichas clínicas de los pacientes que cursaron con alguna infección osteoarticular y se obtuvieron los datos demográficos y clínicos de cada paciente. RESULTADOS: Treinta pacientes fueron incluidos. Dieciséis (53,3%) fueron hombres con un promedio de edad de 38,5años. El 50% correspondieron a infecciones asociadas a la atención de salud. Las principales infecciones fueron asociadas a material de osteosíntesis. El agente más frecuente fue el Staphylococcus aureus meticilinosensible (SAMS). En la mayoría de los casos se realizó aseo quirúrgico, toma de cultivos e inicio de antibióticos con ceftriaxona más clindamicina. CONCLUSIONES: Las infecciones osteoarticulares ocurrieron principalmente en hombres jóvenes, fueron infecciones asociadas a material de osteosíntesis y producidas por SAMS.


INTRODUCTION: Osteoarticular infections are a common challenge in everyday trauma practice. In Chile, and in different hospitals in the region of Valparaiso, there are few studies on the issue, which is essential to optimise both surgical and medical treatment of these infections. OBJECTIVES: To describe the osteoarticular infections in the Hospital del Instituto de Seguridad del Trabajador (IST) of Viña del Mar during the 2012-2013 period. MATERIALS AND METHODS: A retrospective descriptive study was performed on a series of cases. A review was made of the medical records of patients with any osteoarticular infection. Demographic and clinical data were obtained from each patient. RESULTS: Of the total of 30 patients included, 16 (53.3%) were male with a mean age of 38.5 years. Half (50%) of the infections were associated with health care. Major infections were associated with osteosynthesis material. The most frequent agent was Methicillin Susceptible Staphylococcus aureus (MSSA). In most cases surgical cleanliness, taking of microbiological cultures, and antibiotic treatment with ceftriaxone plus clindamycin, was performed. CONCLUSIONS: Osteoarticular infections occurred mainly in young men, were infections associated to osteosynthesis material, and caused by MSSA.

La radiografía panorámica ha adquirido un gran valor diagnóstico por su bajo costo y detección de hallazgos que no son detectables en la clínica. En cirugía maxilofacial son de utilidad en el seguimiento de procedimientos quirúrgicos mayores ya que permiten visualizar los materiales de osteosíntesis y determinar las patologías óseas con indicaciones quirúrgicas. Objetivo: determinar la prevalencia de material de osteosíntesis en radiografías panorámicas de adultos, buscando su asociación con trauma, anomalía dentofacial, patología quirúrgica y defecto óseo. Método: se valoraron 10.000 radiografías, clasificándolas por sexo, edad y diagnóstico presuntivo. Resultados: se encontraron 173 radiografías con hallazgos de fijación rígida en su mayoría mujeres, entre 21-30 años. Los hombres sufren más trauma mientras que la cirugía ortognática se hace más en mujeres. Conclusión: El material de osteosíntesis se asocia más a corrección de anomalías dentomaxilofaciales en mujeres jóvenes, mientras que la fijación interna rígida asociada a patología quirúrgica es de baja prevalencia...


Panoramic radiography has acquired great diagnostic value given its low cost and its capacity to detect findings that are non-detectable in clinical practice. In maxillofacial surgery, they are useful to monitor major surgical procedures as they enable professionals to detect osteosynthesis materials and thus determine bone diseases with surgical indications.Objective: to determine the prevalence of osteosynthesis material in panoramic radiographs of adult patients, looking for its association with bone defects, surgical pathology, dentofacial anomaly and trauma. Method: 10,000 x-rays were evaluated and classified according to sex, age and presumptive diagnosis. Results: there were 173 x-rays with findings of rigid fixation, mostly among women aged between 21 and 30. Men tend to suffer more from trauma, while orthognathic surgery is more prevalent among women. Conclusion: Osteosynthesis material is more often associated with the correction of dentomaxillofacial anomalies in young women, while the rigid internal fixation associated with surgical pathology has low prevalence...
